MDT Substance Use Outreach Worker

This role will provide an assertive outreach response to service users who are not currently engaged in treatment provision and recovery support and or require complex unmet need proactive engagement tools to facilitate access to services and reduce barriers to engaging in treatment. The role will work closely with housing support, homelessness services and Statutory providers of specialist clinical substance use support, and social care.

Location: Base office – Cardiff. Co-location with other social care partners may be required.

37 hours per week

Salary:
Starting at £24,931

Duration:
Permanent, in line with funding
