Mental Health and Wellbeing Coach
This role will support the delivery of a 24/7 support service in partnership with Aneurin Bevan University Health Board (ABUHB). The service will support patients’ transition from secure hospital residential settings into a supported community setting. Referral (SPOC) Coordinator
To facilitate referrals and accurate record keeping for all initial contact points of CAVDAS. The role will monitor the Single Point of Contact CAVDAS email, website ‘contact us’ notifications and Single Point of Contact telephone number for Cardiff and Vale of Glamorgan. They will be responsible for updating and facilitating initial referrals, signposting, and ensuring appropriate response for specialist support and information.
