Mental Health and Wellbeing Coach
This role will support the delivery of a 24/7 support service in partnership with Aneurin Bevan University Health Board (ABUHB). The service will support patients’ transition from secure hospital residential settings into a supported community setting. The service will also aid the transition from CAMHS into Community Mental Health Teams. Eventually, the service will aid the individual to then transitioning into independence within the community.

Referral (SPOC) Coordinator
To facilitate referrals and accurate record keeping for all initial contact points of CAVDAS. The role will monitor the Single Point of Contact CAVDAS email, website ‘contact us’ notifications and Single Point of Contact telephone number for Cardiff and Vale of Glamorgan. They will be responsible for updating and facilitating initial referrals, signposting, and ensuring appropriate response for specialist support and information.

Support Assistant – Nights
We are looking for caring and compassionate colleagues to work waking nights on a two weekly rolling pattern supporting colleagues to enable and assist people aged 16+ who are homeless or at risk of homelessness and have complex support needs to make changes to their lives.
