We are seeking to appoint an enthusiastic and confident social worker to join our active and well established team.
About Us:
Hospice of the Valleys is the specialist provider of palliative care services in Blaenau Gwent. Our specialist teams provided support to over 600 patients last year; in their own homes, in our Day Centre, at Outpatient appointments or in hospital. We provide nursing support, respite care, social work and bereavement support, physiotherapy, complementary therapies and welfare rights support to people with life-limiting illnesses across Blaenau Gwent. At present we also provide bereavement support in Merthyr on a temporary basis.
Job Role:
The successful applicant will work as part of a multi-disciplinary team providing support to Hospice patients and their families and carers in making decisions about their future needs as they adjust to a new and changing situation, offering appropriate interventions.
Use of a car and a full driving licence is essential.
