The Home First Support Service provides personal care and reablement support to older people on a short-term basis as a means of either avoiding hospital admission or supporting discharge from hospital. Home First Support Service Worker x 2

We will help and support older people, especially those who are vulnerable, isolated, or in poverty.

You will be a confident lone worker experienced in delivering personal care and promoting independence, flexible with good communication skills.
The post requires as a minimum a Diploma level 2 in Health & Social Care or equivalent or a commitment to achieve a qualification.
ICT skill are essential although additional training will be provided
The Service operates between the hours of 8am & 6pm, 7 days week.
