Summary:

Age Connects Cardiff and the Vale is looking to recruit for a full time and part time Discharge Support Officer to work within the Integrated Discharge Service (IDS) at the University Hospital of Wales and Llandough Hospital.

  • Recruiting staff and volunteers
  • Being proactive in seeking partnerships to improve the care and support for older people.

Job Role:

Assisting older people over the age of 60, who are in hospital, and their families and carers with discharge planning so an understanding of hospital discharge procedures and protocols along with experience of working in a multi-agency setting is essential.

The post holders will need to work flexibly as part of a large integrated team of health and social care professionals in a busy environment.

1 post 21 hours at University Hospital of Wales £12,149 and 1 post 35 hours at Llandough Hospital £20,248 * (funded until March 2021)

Both posts will be subject to a satisfactory Enhanced Disclosure check.
