Age Connects Cardiff and the Vale is looking to recruit for a Discharge Support Officer to work within the Integrated Discharge Service (IDS) at the University Hospital of Wales and Llandough Hospital.

Job Role:

Assisting older people over the age of 60, who are in hospital, and their families and carers with discharge planning so an understanding of hospital discharge procedures and protocols along with experience of working in a multi-agency setting is essential.

The post holder will need to work flexibly as part of a large integrated team of health and social care professionals in a busy environment.
