About Us:

The Bridgend Carers Centre was established nearly thirteen years ago following needs identified throughout the Bridgend County Borough area. The project is core funded by Bridgend County Borough Council and Abertawe Bro Morgannwg Health Board.

The Centre continues to be supported by the Carers Trust and is a part of the network of Crossroads Schemes and Carers Centres throughout the UK. As part of the national network, Bridgend is able to take advantage of a range of opportunities for carers as well as support and training for members of staff.

The organisation has charitable status and is a Company Limited by guarantee, managed by a Board of Directors. The Centre Manager has a responsibility for the development of the services, the management of the staff and ensuring the work of the project meets the requirements of all its the funders.

Carers throughout the County Borough area are able to access support from the Centre, by self-referral or via any other agency. The service is provided for any adult carer of any age, regardless of the age of the “cared for” person. There is a separate service locally to support young carers, and both organisations work co-operatively together to meet the needs of the families.

The aim of the Centre is to offer a range of services for carers which enables them to receive appropriate support, prevent crises, to preserve their health and well being, to help relieve social isolation and to acquire skills to meet the needs of their different caring situations. Ensuring that carers can easily access the right information at the right time is also a key part of the Centre’s work. There is also a responsibility to target hidden carers, raise public awareness about the issues facing carers and to involve carers in the planning and development of services.

Members of the Carers Centre staff team work with a wide range of local voluntary organisations, and carer networks as well as maintaining close working relationships with colleagues in health and social services. There is also involvement and participation in key multi-agency strategic planning and partnership groups within the County Borough area to ensure that carers remain on critical agendas. Carers Link Support Worker x 3

Job Role:

Carers Link Worker will work proactively with GPs and District Nurses in each network to create a new pathway of referral and improve carer recognition, including young carers, to identify people who have become carers due to the health of their cared-for and to give information, advice and assistance to these newly identified carers.

Additional Information:

The post holders will work as link officers for carers within each of the 3 Integrated Network teams.

You will Hold a full UK Driving Licence and have access to a car.

These posts are initially funded until March 2019
