Summary:

The Healthy & Active Partnership project is to reduce isolation amongst older people in need of help through making full use of the services and activities available in their community or alternatively through the deployment of a volunteer support program.

About us:

Age Connects Cardiff and the Vale helps thousands of local older people. Help is at hand when people most need it maybe coming home to an empty house after a stay in hospital, being supported when making difficult choices such as moving into a care home. For many housebound older people their weekly volunteer visitor may be the only person they see to talk to and spend time with.

Job Role:

Specific Objectives of the post

     To promote the service to help spread awareness of the project and most importantly recruit volunteers.

     To assess client’s needs in terms of isolation and then provide them with a range of solutions to reduce their isolation.

     To develop a relationship with local services and community activities that an isolated older person might benefit from attending.

     To manage a large team of volunteers who are either providing support in the client’s own home or in the community.
