The Home First Support Service provides personal care and reablement support to older people on a short-term basis as a means of either avoiding hospital admission or supporting discharge from hospital.
To live in a society where older people are respected and enabled to meet their aspirations.
We will help and support older people, especially those who are vulnerable, isolated, or in poverty.
Baroness Finlay came to live in Cardiff 1976 and is a past president of the Royal Society of Medicine. She is a professor of palliative medicine at Cardiff University School of Medicine, and is consultant at the Velindre Cancer Centre in Cardiff. In 2001, she was made a life peer as Baroness Finlay of Llandaff.

In our society where some older people are still being denied basic care and respect, joining forces to work towards lasting solutions is paramount.”
This is an exciting opportunity for some-one who wants to further their career within the domiciliary care environment.
You will be an experienced care worker with a level 3 diploma in health and social care and have a good understanding of completing client assessments, reviews and service delivery plans along with the ability of supporting and mentoring a small team.
The ability of organising and prioritising your own workload and that of others is essential.
The post holder will need to work flexibly in a busy environment with excellent communication and IT skills.

A full valid driving license and use of a car is required for both posts as there will be extensive travel across Cardiff and the Vale.
